If you're a first-time client, please review and fill-out the client intake form as much as you can and bring it to our first session.

Electronic Payment Authorization Form
If you wish to use a credit card as form of payment, please complete the Electronic Payment Authorization form. I will keep this form securely in your clinical file. This will allow me to bill for services outside of my time with you; that way, our sessions are not disturbed with this activity.
