If you're a first-time client, please review and fill-out the client intake form as much as you can and bring it to our first session.

Electronic Payment Authorization Form
If you wish to use a credit card as form of payment, please complete the Electronic Payment Authorization form. The following forms of payment are accepted: Visa, MasterCard and Discover. Service fees will be deducted from the designated account at the time services are rendered. The debit or credit card information you provide on this form will be destroyed once your first payment has been made. This will allow me to bill for services outside of my time with you; that way, our sessions are not disturbed with this activity.
